Commonly used high school forms:

Class Change Form – Students in the beginning of the school year can request class changes with this form.

College Visit Form – Students use this form to coordinate and approve an exused absence for a college/career visit.

Teacher Recommendation Request Form –  Students can use this form to coordinate with teachers for letters of recommendation for college.

FRCS 2017-2018 Transcript Release– Students submit this form to authorize release of information after electronic notification is received by the school (usually to be released via email or online application)

Transcript Release Form – Paper – Students and graduates submit this form to authorize release of information to be mailed.

Graduate or Former Students

Transcript Release Form – Graduate or Former Student 2017 – Graduates and former students, submit this form for transcripts.
